Delta Dental PPO
Delta Dental does not issue dental ID cards. When you check in for an appointment, simply provide the PUSD
Group #6779-0001 and the employee’s Social Security Number.
This plan gives you the flexibility to see any dentist, but you’ll pay less out-of-pocket when you use an in-network provider. If you see a provider out-of-network, you’ll be responsible for any charges above the allowed amount, and you may need to file a claim.

MetLife Dental HMO
With the MetLife Dental Health Maintenance Organization (DHMO) plan, you must select an in network general dentist to manage your care. All services, including routine check-ups and emergencies, must be provided by your chosen dentist.
Visit www.metlife.com to search for a participating dentist in your area. Follow these steps:
• Select “Find a Dentist.”
• Choose “Dental HMO/Managed Care.”
• Enter your city or ZIP code and click “Find a Dentist.”
• Select MET50 as your plan option, then click “Go.”
• When you select a dentist, write down their facility number—you’ll need it when enrolling in Selerix.
Dental ID Card:
This plan includes a dental ID card. Make sure the card lists the dentist you selected during open
enrollment. If the information is incorrect, contact MetLife at 800-880-1800 for assistance. Keep your card and bring it to your appointments.
If you did not choose a dentist when you enrolled, an ID card will not be issued. You will need to contact
MetLife to update your provider selection.
