Alternate Transportation Form
If student athlete will not be transported with the team, this form is required to be completed and turned in to Athletics the day prior to the event. Also to be completed when 'Private Vehicle Transportation' form T-30 is used.

Athletic Clearance Instructions
Instructions to register athletes via We encourage all parent/guardian's and their athletes to register at for online athletic clearance to minimize errors, better facilitate the Ticket to Play process, and avoid delays for the upcoming school year.

Athletic PE Exemption Form
For 10th-12th grade students only. Students enrolled in any PE class are not concurrently eligible for a PE exemption. It's the student's responsibility to fulfill the sport's requirements {as designated by the coach} and to turn in the paperwork by the deadline. It's coach's responsibility to notify Athletics that the student fulfilled the season with the team.

The fall deadline is August 31, 2021 - Cheer (Sideline), Cross Country, Field Hockey, Football, Golf (Girls), Tennis (Girls),  Volleyball (Girls), and Water Polo (Boys).

The winter deadline is November 30, 2021 - Basketball (Boys, Girls), Cheer (Stunt), Roller Hockey, Soccer (Boys, Girls), Water Polo (Girls), and Wrestling (Boys, Girls).

The spring deadline is March 4, 2022 - Badminton, Baseball, Golf (Boys), Lacrosse (Boys, Girls), Softball, Swim & Dive, Sand Volleyball (Girls), Tennis (Boys), Track & Field, and Volleyball (Boys).

Complete this form once you have been named to a team's roster, and turn it into Athletics dropbox by the deadline noted. Drop off the forms school days Monday – Friday, between 8 a.m.-3:45 p.m.  {no appointment necessary}.  *There will not be drop-off availability during non-student days/District breaks/holidays.* 

Authorization for Student to Carry Medication
If student uses any medication during the school day and/or while playing sports, parent/student/prescribing health care provider must complete PUSD Form H-26B {MAF}; however, students with anaphylaxis, allergy, and inhaler will use PUSD Form H-58 {AAEP} {required beginning in the 2019-2020 school year}. Bring the medication for verification along with the completed form to the Athletic Assistant prior to fall tryout, or a week before the beginning of each school year.

Health History and Physical Exam Forms
Complete these forms prior to attending Physical Night, seeing your pediatrician, or registering your athlete via Clearance is required annually/seasonally for student athletes to obtain their Ticket to Play. Physicals are good for ONE year from date of examination and cannot expire during the sports season. {Must be valid prior to the season and valid through the conclusion of the season.}

Athletic Insurance Requirement
In compliance with California Education Code 32221, you must certify that insurance coverage exists for your son/daughter and that this coverage will remain in effect throughout the time that he/she participates in sports. One option for your consideration is Pacific Educators, or via (800) 722-3365.

Off Roll Recommendation by Season of Sport
This document lists the sports that it is recommended the student take an off roll so that they do not miss fourth period class on a regular basis. 

Physical Night Information

Due to restrictions related to Covid19, we will not be able to have our Physical Night annual fundraiser during the spring of 2021. We look forward to scheduling it again in spring 2022!

Private Vehicle Transportation
When transportation is not provided for the team, Parent/Guardians interested to volunteer to drive athletes (other than their own athlete) are directed to the Westview Receptionist to complete the Volunteer Paperwork in person. The Receptionist will either confirm Parent/Guardian are in the PUSD Volunteer database, or Parent/Guardians must complete the Westview Volunteer Paperwork including Level I - Megan's Law clearance, Code of Conduct, and provide a valid TB test result prior to your Private Vehicle Transportation Form being approved.

Once notification of Level 1 - Megan's Law approval is received by the Westview Receptionist, Parent/Guardian will be invited to provide the Private Vehicle Transportation Form (T-30), declarations page from vehicle insurance provider showing required minimums, and valid California drivers' license to the Athletic Assistant.

Ticket to Play
The student athlete will be eligible to obtain a Ticket to Play (TTP) when the Athletic Clearance steps are completed prior to the due dates noted each season, and student status is Cleared. Your PUSD ID card is required to receive your Ticket to Play.

Alternate Athletic Packet

We strongly encourage using AthleteClearance. If you select "alternate athletic packet" option, clearance and/or tryout could be delayed. If you have registered at AthleteClearance, "alternate athletic packet" is not required.
