Athletic Resources and Forms

 
These are the forms to print and bring to your doctor when obtaining a pre-participation physical exam (Sports Physical) as part of the Athletic Clearance requirement​.  For the 2023-2024 school year, completed and signed hard copies must be turned into the Athletics office.  
Please note that Sports Physicals expire one year from the date of the exam.  Physicals that expire during a student-athlete's season of sport WILL NOT be accepted...no exceptions!

Per California Eduction Code 32221, all student-athletes must show proof of medical insurance coverage that will remain in effect the entire time the student is participating in MC athletics. If a student is not currently covered, one option to consider is Pacific Educators at www.peinsurance.com/sign up or (800) 722-3365. Coverage with Pacific Educators must be renewed each year.
