Ticket to Play and Physicals
June 1, 2022 in the Nighthawk Gym
  1. Purchase your ticket on "Go Fan" website (limited tickets available)
  2. Print and complete the Health History Form (2 pages) (click here for forms)
    Print the Physical Exam Form
  3. Bring the forms with you on June 1st
    Times are for grade levels during 2022-23 school year:
  • 9th grade: 5:30pm
  • 10th grade: 6:15pm
  • 11th grade: 7:00pm
  • 12th grade: 7:30pm 

Ticket to Play/DragonFly Max - All students interested in playing a sport must complete this process.

Proceeds from Physical Night help the athletic trainer buy necessary supplies/equipment to help with athletic injuries next school year. 

Important Dates
  • P.E. Exemptions - For students participating in CIF High School Sports.  Click Here!  
  • OCIS Applications - For students involved in an individual, non-school related Sport. Fall 2021-2022 (Tri-1) applications are due by April 2, 2021 by 3:45pm. OCIS information can be found on the PUSD Website at Alternative Programs.  Drop off at the front office of Del Norte or scan and email the completed packet to Mike Giaime
