2013-14 Information and Forms
Forms for New Student Enrollment for the 2013-2014 school year are available at the PUSD Enrollment link beginning May 1st, 2013.
Kindergarten Enrollment - Begins May 8, 2013
Kindergarten Enrollment begins Wednesday, May 8th, from 8:30 am to 3:00 pm daily and continues through June 21st. On May 8th at 2:30 pm we will have a Kindergarten Orientation in our Multipurpose Room.
Prior to arriving please visit the PUSD District website at www.powayusd.com to download the 2013-2014 forms needed to enroll your child. Please don't forget that ALL children enrolled in Kindergarten or Transitional Kindergarten MUST have had their 5 year immunizations, otherwise their enrollment will not be accepted, regardless of age. Our Kindergarten teachers have put together Kindernotes to help answer any specific questions you may have regarding Kinder Rules At The Ranch. Again, please join us at 2:30 pm on Wednesday, May 8th for Kindergarten Orientation. There will be time for questions from our K Team. They look forward to meeting you!
1st-5th Grade New Student Enrollment - Begins May 13, 2013
Enrollment for students entering 1st - 5th Grade for the 2013-2014 school year begins May 13th, from 8:30 am - 3:00 pm daily and continues through June 21st. Enrollment forms are available at http://www.powayusd.com/enrollment/. Please note immunization requirements below; students will not be permitted to enroll until these requirements are met. In addition, you must bring with you the child's birth certificate or passport, two separate forms of residency verification, and the completed immunization records.
During July, new student enrollment for Transitional Kindergarten - 5th grade will be taking place at the District Office. Check back for exact dates. 2013-2014 Supply Lists for 1st-5th Grades
REQUIRED DOCUMENTS TO BRING FROM HOME AT TIME OF ENROLLMENT
þ Completed 2013-14 enrollment packet which will be available online May 1st.
þ Birth Certificate, Baptismal, or
Passport (the original and a copy).
þ Original Immunization Record (the original and a copy). All immunizations need to be up to date.
þ Two (2) separate forms of residency verification
(original and a copy) (Examples:
deed to primary residence, lease agreement, utility bill.)

Volunteer Forms
Volunteer forms and information can be found at the Volunteer District Site.
PLAN AHEAD…Have immunizations completed.
•4 Doses - Polio (3 doses if at least one dose was given on or after fourth birthday)
•5 Doses - DPT (4 doses if at least one dose was given on or after fourth birthday)
•2 Doses - MMR (Both doses must be given on or after first birthday)
•3 Doses - Hepatitis B
•1 Dose - Varicella (Or health care provider documentation of chicken pox disease or immunity)
Official immunization record must include:
• Name of Child
• Date of Birth
• Name of immunization administered
• Month and year of EACH immunization. Include day if obtained within birth month.
• Name of health care provider or agency administering EACH vaccine.
If you are taking your child to the doctor for their five year old physical and immunizations prior to enrollment, you may download the Report of Health Checkup for School Entrance MCH77 from the Poway Unified School District website (http://www.powayusd.com/enrollment/) under new student enrollment. The Oral Health Checkup Form is also available on the website.
