Please take a few minutes at home to carefully check your child’s health before he/she leaves for school each morning.

Greetings Explorer Families,  

Welcome to the 2014-2015 school year! It is hard to believe that we are now beginning our seventh year. The staff and I are excited about continuing to create an optimal learning environment for all of our students.  

August 18th 9-12pm and 4-6pm

All 1st through 5th Grade Continuing Student Registration - MPR 

Come and update the annual student registration information and find out your child’s class placement for the year.

You may fill out the forms below and bring them with you.  All forms will also be available at the time of registration.

If you have moved, provide two copies of your new residency information.

Bring a copy of any updated Immunization Records

Signature Verification Form  All 4th and 5th grade students need to sign

Review of Student Handbook

Student Health Information -4th Grade only unless there is a change of new health concerns/medication
