We are happy to welcome you to Twin Peaks Middle School!
New Student Enrollment for Poway Unified School District is an on-line process. For new enrollment for the 2020-2021 school year, please contact your School of Residence at the appropriate enrollment email address noted below. As needed, please use the Address Lookup tool to assist you with the confirmation of your correct School of Residence.
PLEASE NOTE: Your student’s enrollment ensures a seat at a school in Poway Unified. We make every effort to enroll your child at your neighborhood school. In some cases, classes at your neighborhood school may reach capacity, in which case we will work with you to place your child at a nearby school.
STEP 1: TWIN PEAKS INTAKE FORM:
Please complete the New Student Enrollment Intake form for TWIN PEAKS MIDDLE SCHOOL This form will take approximately 20-30 minutes to complete and you will be required to upload a copy of your Government Issued Photo ID, your student's age verification** and residency verification documents [See Residency Verification Documentation].
** Original Birth Certificate, Current Passport or Baptismal Certificate (TK-12).
- Parent's or Guardian's Government Issued Photo ID
- Student's birth certificate or Passport
- two proofs of residency (e.g., Mortgage, Lease Agreement, Utility Bill, etc. with Address)
- Immunization records (see Health Services for specifics)

Please ensure you allow yourself enough time to complete the form and have the necessary documents ready to upload prior to beginning this step.

STEP 2: You will be contacted by your school site regarding next steps to complete the enrollment process.

STEP 3: Prepare Middle/High School Required and Optional enrollment forms, which you can find below to be uploaded in Step 4.

STEP 4: Per directions from your school enrollment team, upload Middle/High School Required and Optional enrollment forms (below). If possible, registration forms and other documentation should be submitted in PDF format.

STEP 5: Upon receipt and review of Required Enrollment Forms, you will be contacted by your school site in regards to enrollment confirmation and placement.

MIDDLE /HIGH SCHOOL REQUIRED FORMS
IMPORTANT: If you do not reside in Twin Peaks Middle School's boundaries, you will also need to apply for an Intradistrict transfer (within PUSD) district form PP-126 or an Interdistrict transfer (outside PUSD) SDCOE form 341. These forms are submitted to the PUSD Office located at 15250 Avenue of Science, San Diego, CA 92128-3406, not the school site. LOGIN INFORMATION
Once your student is enrolled, you will be given a student ID number for your student. Your next step is to set up a parent account to allow access to
myConnect.
Your parent login will be the student ID number with the letter ‘P’ in front - P{student ID} (ex. P1234567). You must contact the
Technology help desk or
pusdithelp@powayusd.com to receive a password.

*** Helpful but not required: most recent progress report or report card and any Standardized test records or GATE certification (if applicable).

Special Circumstances:
- If your student has been in Special Education (in resource or speech), please bring a copy of the latest Individual Education Plan (IEP) with you to your enrollment appointment.
- If you are NOT the parent or court-appointed guardian, additional documentation/forms are available at the school.
- If your student is a Special Day Class student, please contact our District Special Education office at 858-668-4100.
Once your student is enrolled, you will be given a student ID number for your student. Your next step is to set up a parent account to allow access to myConnect.
