2017-18 New Student Enrollment Information

Enrollment Please note if your student was previously enrolled in a PUSD school in the past but left the District, they are considered NEW and will need to complete the New Student enrollment process. 
  
 
 

Registration is for continuing students (who attended PUSD last year) and is a separate process that takes place every year during the week before school starts in August. (See 2017-18 Continuing Registration Announcement) 

To complete enrollment forms, please go to www.powayusd.com/enrollment, then download and fill out the required forms.

Also, please print out and complete this MVMS Site Specific forms:

You will need to set up an appointment through Counseling Assistant Trish Weaver, who handles enrollments and transfers, after August 1st when the MVMS office reopens for the 2017-18 school year.


When you come to your appointment, please bring:
  1. All completed, signed forms  (please do not staple or print back to back)
  2. Two (2) current proofs of your residency
  3. Updated immunization records
  4. Original birth certificate, passport or baptismal record
  5. Also helpful, but not required: the final report card or transcript from the last school and GATE certification (if applicable)
IMPORTANT: Please check to make sure that your address is in our attendance area by using the address look up tool:  Click Here

If you do not reside in Mesa Verde Middle School's boundaries, you will first need to have an approved transfer. 
  • To apply for an Intradistrict transfer (within PUSD) or an Interdistrict transfer (outside PUSD), you will need to download the transfer form.  It is available on the powayusd.com website “Transfer Requests”. Enroll your student in your home middle school while awaiting the letter from PUSD approving or denying your transfer request. Mesa Verde will not know the status of your request.  Note: We cannot enroll your student at our school until your application (Intradistrict or Interdistrict transfer) has been approved.
Special Circumstances:
  • If your student has been in Special Education (in resource or speech), please bring a copy of the latest Individual Education Plan (IEP) with you to your enrollment appointment.
  • If your student is a Special Day Class student, please contact our District Special Education office at 858-668-4100 before you set up an enrollment appointment.
  • If you are NOT the parent or court-appointed guardian, additional documentation/forms are available at the school.
