Health and Safety Agreement in preparation for in-person instruction

 As we prepare for the return of students to in-person instruction at Meadowbrook, we ask that all parents/guardians review Poway Unified School District’s Reopening Guidebook 2.0. The Reopening Guidebook details health and safety protocols and practices designed to ensure a safe learning environment for staff and students. In addition, we ask that the parents/guardians of students opting to engage in in-person instruction review, sign, and return a copy of the PUSD Parent/Guardian Health and Safety Agreement for In-Person Instruction for the 2020-2021 School-Year. One signed Health and Safety Agreement must be completed and returned to Meadowbrook for each student scheduled to return to campus for in-person instruction. 
 
There are several ways that families can submit a signed Health and Safety Agreement:
 Parents/Guardians may sign and return the document by:
  1. Submitting via a drop box located outside the Meadowbrook main office (beginning Thursday, January 7th from 8:00am-3:00pm and every day thereafter until our first scheduled day of in-person instruction). 
  2. Submitting via drive-through at our Chromebook distribution event, scheduled from 8:00am-3:00pm on a Date to be determined.
  3. Sending a signed copy along with your student(s) on the first day of in-person classes. 
  4. Scanning a signed copy and emailing it to our Meadowbrook school secretary, Mrs. Priscilla Simmons, at psimmons@powayusd.com.
