Athletic Clearance

AthleticClearance.com registration for the 2020-2021 school year is now open! 
Registration is required for an athlete to obtain a Ticket to Play {TTP} to tryout for a Westview sport team. Follow the Instructions for Completing the Athletic Clearance. If trying out for multiple sports, click here.

Obtain a Physical

Print the Westview's Athletic Physical forms (3 pages). Complete the health history and COVID19 questionnaire, then take these three forms to your doctor. Physicals are good for ONE year from date of examination and cannot expire during the sports season. {Must be valid prior to the season and valid through the conclusion of the season.} Make sure all line items {‘yes’ explanations, dates, signatures, etc.} are completed before you leave the doctor; otherwise omissions may cause delays.

If the athlete needs to take or have medication readily available while at school or during sports, an Authorization to Carry Medication Form H-26B {MAF} or the Allergy and Anaphylaxis Emergency Plan Form H-58 {AAEP} is required with signatures of student, parent, and prescribing physician {valid for the school year; shared with Athletics and Health Office}. Required annually per California State EdCode.

As of this writing, we understand all medical offices are not open for sports physicals. When available, contact your primary care physician to schedule a physical. Stay tuned to this site for updated deadlines and form acceptance schedule.  

Turn in to Westview Athletics

  • we anticipate that we will begin accepting forms on July 20, pending CIF and PUSD guidelines. This date could be moved later, but will not be moved earlier.
  • the signed confirmation page, or pages, if trying out for multiple sports, from AthleticClearance.com, and
  • the completed health history, COVID19 form, and
  • physician signed physical exam form. Physicals can only be accepted from a California Licensed Health Care Provider and are valid for 12-months.
    • If you are a fall athlete, and you believe your physical on file at Westview is valid through fall, you will turn in the confirmation page and COVID questionnaire.
  • If applicable, include the MAF or AAEP, and medication for verification prior to fall tryout, or at the beginning of the school year.

Athletic Clearance 2020-2021 Due Dates

Please check back for season specific due dates as CIF and PUSD make information available. Westview Athletics is closed during summer and Athletic Clearance forms will not be processed. This year we will not have the ability for you to drop off forms over the summer break.

Check Your Cleared Status

Once we start accepting forms you will be able to:
Log into your AthleticClearance.com account. If status reads Cleared, refer to TTP pick up schedule; Uncleared status indicates your registration is pending review; and if you have Denied status please check your email associated with AthleticClearance.com account for action items. The email subject will be titled 'Student Clearance' and the email message will be from Home Campus.

The Athletic Clearance must be renewed each school year. Only Westview Athletic Clearance forms are accepted.
