Athletic P.E. Exemption/Waiver 
Important Things You Need To Know

 

P.E. waiver paperwork can't be issued by coaches or counselors.  The athlete must attend a mandatory lunch meeting to be eligible for a P.E. waiver.  All documents are handed out during the meeting.  The Athletic Director or Administrative Designee handles P.E. waivers.  Please contact the Athletic Director for individual questions.  Please read below for important additional information.
 
  1. You must attend a mandatory meeting during one of the in-season dates listed below to receive your P.E. exemption form.
  2. You must fill out your P.E. exemption form correctly and return it to the Athletic office by 3:45 p.m. on the designated due date. Any late forms will NOT be accepted under any circumstance. 
  3. You do not qualify for an athletic exemption form if you are currently in a P.E. class, on OCIS-PE, or a Freshman.
  4. Academic ineligibility, excessive absences and/or injury may disqualify the exemption.
  5. It's the student’s responsibility to fulfill the sport’s requirements and to return completed paperwork by deadline dates.
  6. For Trimester 3, NO Seniors can apply for a P.E. waiver.
  7. Minimum of 6 hours per week participation in practice and/or games.
  8. Minimum of 12 weeks of participation.
  9. Attended a minimum of 90% of all practices and activities based on coaches' attendance records.
Mandatory Athletic Exemption Meeting Dates for 2019-2020
 
  • Fall SportsWednesday, September 4, 2019 or Thursday, September 5, 2019 at the beginning of student lunch in the Nighthawk Center.  Meetings will start promptly 5 minutes after lunch bell. If you are late, you will not be given a form. The deadline to turn in the form for a P.E. waiver is Thursday, September 12, 2019 at 3:45 p.m. NO late forms will be accepted!
  • Winter Sports- Wednesday, November 20, 2019 or Thursday, November 21, 2019 at the beginning of student lunch in the Nighthawk Center.  Meetings will start promptly five minutes after the lunch bell. If you are late, you will not be given a form. The deadline to turn in the form for P.E. Exemption is Thursday, December 5, 2019 at 3:45 p.m. NO late forms will be accepted!
  • Spring Sports- Wednesday, February 26, 2020 or Thursday, February 27, 2020, at the beginning of student lunch in the Nighthawk Center.  Meetings will start promptly 5 minutes after lunch bell. If you are late, you will not be given a form. The deadline to turn in the form for P.E. Exemption is Thursday, March 5, 2020 at 3:45 p.m. NO late forms will be accepted!
