2018 Ticket to Play Information and Other Forms
Parent Driver Forms/ Information Needed (after approval from the coach):
If you're planning on playing a sport at Del Norte High School, you must have a Ticket to Play.
Important things to know:
- Deadlines are firm.
- If you're scheduling your own doctor's appointment, we encourage you to schedule it on or after May 30, 2019 to ensure the physical is good through the school year.
To get your "Ticket to Play," print and fill out all pages of the athletic packet (5 pages). Forms need to be updated each school year, must be filled out in pen (not pencil), and all forms must be submitted together.
Physicals are valid for 12 months from the date of exam and must not expire during the athletes' season of sport, including tryouts.
Turn in forms to the
athletic department only in the administration office of DNHS. Please read the athletic handbook before you complete the forms required to receive your "Ticket to Play." Athletes and parents/guardians need to understand and follow the information contained in the handbook.
Student Insurance: Insurance is required for all students trying out for sports. If you are not currently covered, student insurance may be purchased through Pacific Educators, www.peinsurance.com/sign up. Bring in either a completed application with a check or proof of online purchase with the Ticket to Play paperwork.
