Ticket To Play Paperwork

Beginning in May 2019 for the 2019-2020 school year, the Athletic department will use an online system called DragonFly MAX for Ticket to Play paperwork.
Click on the Dragonfly logo above to fill out paperwork before Physical Night on Thursday, May 30, 2019.
Parents need to register and create a profile for their child first, and then the athlete may register. 
Detailed Parent Sign-up Instructions for DragonFly MAX

If Attending Physical Night:

  • Complete the required documents through Dragonfly MAX (the tab at the top of the website page directs you to each section). You must print the Medical History/Physical Form (two pages), complete the Medical History form, and then take both the completed Medical History form and Physical form to Physical Night along with the DragonFly MAX Completion page for proof that you have completed all required sections online. You will bring three pages to Physical Night (the Medical History form and the Physical form {may be two-sided} and the DragonFly Completion form).
  • If the athlete needs to take or have medication readily available during any sporting events (practices and/or games), an Allergy and Anaphylaxis Emergency Plan Form H-58 is required for Epipen/antihistamine medications. An Authorization to Carry Medication Form H-26b is required for other medications needed if the Allergy and Anaphylaxis Emergency Plan Form H-58 is not filled out. Both forms must be signed by the prescribing physician, a parent, and the student and turned in to the Health Office and Athletics department. These forms are valid for the whole school year.
Del Norte Sports Medicine will accept any gently used crutches, arm slings, braces (ankles, knees) etc. if you would like to donate them on Physical Night.

If NOT Attending Physical Night:

  • Complete your child's Ticket to Play paperwork through DragonFly MAX.
  • Print the Medical History/Physical forms along with the DragonFly MAX Completion page (total of three pages--the Medical History form and Physical form may be two-sided, and the DragonFly MAX Completion form). The Medical History and the Physical forms must be taken to a medical professional and completed.
  • If the athlete needs to take or have medication readily available during any sporting events (practices and/or games), an Allergy and Anaphylaxis Emergency Plan Form H-58 is required for Epipen/antihistamine medications.   • Submit the Medical History/Physical forms and the Dragonfly MAX Completion
  • page to the Athletics department after your child has seen their healthcare provider. See below for Ticket to Play submission dates.
  • Physicals must be dated on or after 5/30/19 or during June/July to be valid for the entire 2019-20 school year.

Ticket to Play 2019-2020 Submission Timeframe

  • June 3 - June 20, 2019 for Fall Season Sports (Dance/Cheer - contact coaches)
  • October 14 - October 25, 2019 for Winter Season Sports
  • January 13 - January 24, 2020 for Spring Season Sports
Failure to complete DragonFly MAX paperwork and submit the Physical forms may delay and/or prohibit the athlete from trying out.

Ticket to Play Blue Card Pick-Up

-  Once the above steps have been completed prior to the due dates listed for each season, athletes may be cleared and eligible to obtain their Ticket to Play blue card.
-  If your athlete is cleared to participate in any sport, the athlete must pick up their Ticket to Play blue card only during specified dates/times in the Athletics department (see Timeframe sheet above), and give it to their coach on the first day of tryouts.
-  Athletes trying out for more than one sport throughout the year must pick up their Ticket to Play blue card again during specified dates/times in the Athletics department.
No Ticket, No Tryout, No Exceptions!

Check your Cleared Status

You can log in to DragonFly Max as often as you'd like to update important information, check the paperwork clearance status of your athlete, and check injury status throughout the season.
Please contact the Athletic Trainer if your child has an injury you are concerned about.

NOTE: We only accept original, hard copy physicals that are signed, dated, and stamped by a CA Licensed Healthcare Provider.
→ Physicals are valid for 12 months and cannot expire during your sport season.
→ If the student is transferring from another school, parents should ask for the athlete's physical from that school and bring it to Del Norte Athletics department along with the Dragonfly MAX Completion page.
→ Ticket to Play paperwork in DragonFly MAX must be completed each school year.
