Abraxas High School 2018/19 Registration

Diploma Students: July 17 - July 18  
Registration Drop-Off : 7:30am-3:00pm
 
 What you need to do:
· Download, print, read and sign each of the Registration Mandatory Forms.  If you are unable to print the forms you may do so in
  the Library during registration hours.

· Please double check for all signatures and paperwork as we only accept completed forms.  Any paperwork missing signatures or
  pages will be returned until full completion.
· Drop off ALL Registration Forms anytime between 7:30am-3:00pm on July 16th or 17th.
· If you have any questions please call the Abraxas Office at 858-748-5900.

Student Health Information - Please read, complete and sign.  If there are no health concerns the bottom box.
**If your student needs to take any medication at school please see the Health Tech for the medication permission form.
Abraxas Student Enrollment Contract - Parent and student signatures required
Abraxas Signature Verification of Receipt of Documents - Please read, complete and sign.  Students are required to sign in two
   places.
Abraxas Student Services Education Support Group Permission - Parent signature required
Abraxas Presentations/Trainings Permission – Parent signature required
Abraxas Video Permission - Parent signature required
PUSD Mass Notification Opt-In Form - Please read, complete and sign.
PUSD Signature Verification - Please read, complete and sign. Students are required to sign this form in two places.

Proof of Residency
If you have moved, please print the Proof of Residency form, complete, sign, and provide copies of two proofs of residency from the approved list.  Bring those with the rest of the forms.
