Transition Program

WHAT IS THE TRANSITION PROGRAM?

Since 1986, the Transition Program in the Poway Unified School District has operated as a community-based program designed to enhance and promote greater independence of practical life skills for young adults with developmental disabilities. 
 
The program is located on the Abraxas Continuation High School campus and serves as a link between the public school system and the world of post-school activities and services.

The Transition Program provides training for individuals between the ages of 18-22 across Education/Training, Community, Vocational, and Independent Living domains. 

Students are encouraged to be as independent as possible in the community through the development of transition services and subsequent goals outlined in each individual's Individualized Education Program, including the Individualized Transition Program.
 

OUR MISSION

Our mission is to provide young adults with disabilities within our school district with both classroom instruction and community-based educational opportunities while enhancing and promoting greater independence of practical life skills.

CRITERIA FOR ENROLLMENT

ELIGIBLE STUDENTS:
  • have not received a high school diploma
  • live within Poway Unified School District boundaries
  • are recommended and determined to be placed by IEP team
  • must be between the ages of 18-22 years of age
