Continuing Student Registration
for the 2017-2018 School Year

If your child will continue to attend Turtleback Elementary for the 2017-2018 school year, there are several forms that require your attention. Your child will be bringing them home the week of May 8th.

Please review, complete, and sign the forms in the packet and return the packet to your student’s teacher no later than May 19th.

Student Profile Form: If anything has changed, please make corrections on the form and make sure to sign it.
  • Note: The Student’s home language cannot be changed.
  • If your address has changed, you will need to fill out a new residency verification form and provide two proofs of your new address. If you are now living outside of Turtleback boundaries, you will also need to complete an IDT form as well. Please come to the school office to obtain the necessary forms.
  • You are encouraged to have at least two emergency contacts listed.

Signature Verification Form: Please sign where indicated on the right side of the form. Students going into 4th and 5th grade will need to sign this form as well. Visit for the read only documents referred to on the Signature Verification form. The read only documents do not need to be printed or returned.

Please be aware that not all packets are the same. They differ as follows:

Once you have returned the completed package, you will be registered for the 2017-18 School year at Turtleback.

Please let the office know as soon as possible if your plans have changed and you will NOT be returning to Turtleback or if you have moved and need to update your address.
