Continuing Student Registration
for the 2019-2020 School Year
IMPORTANT NOTICE FOR CONTINUING STUDENTS FOR THE 2019-2020 SCHOOL YEAR:
If your child will continue to attend Turtleback Elementary for the 2019-2020 school year, there are several forms that require your attention. It will be completed ONLINE this year, details to follow.
Student Profile Form: If anything has changed, please make corrections on the form.
- Note: The Student’s home language cannot be changed.
- If your address has changed, you will need to fill out a new residency verification form and provide two proofs of your new address. If you are now living outside of Turtleback boundaries, you will also need to complete an IDT form as well. Please come to the school office to obtain the necessary forms.
- You are encouraged to have at least two emergency contacts listed.
Signature Verification Form: Please sign where indicated on the right side of the form. Students going into 4th and 5th grade will need to sign this form as well. Visit
/registration for the read only documents referred to on the Signature Verification form. The read only documents do not need to be printed or returned.
If you feel that there is information regarding your student that would be helpful for their placement next year, please complete and return this form

Once you have submitted the completed online package, you will be registered for the 2019-2020 School year at Turtleback.
Please let the office know as soon as possible if your plans have changed and you will NOT be returning to Turtleback or if you have moved and need to update your address.
