Required: Health and Safety Agreement for In Person Instruction

As you know your child’s class will return to campus on Monday, October 12th.  Our district has created a Parent/Guardian Health and Safety Agreement for In Person Instruction. This must be completed prior to your child’s return to campus.    The Health and Safety Agreement reviews the components PUSD has put together for a safe return to school including: PUSD reopening guidelinesAT HOME required daily health screening and cleaningSchool screeningsRequirements of face coveringsThe procedures and practices for symptoms and preventing the spread of Covid-19Quarantine responsibilitiesVisitors on campus Attached please find the 4 page document. Please read it carefully, detach and sign the last page, including all lines. We will be accepting these forms on Thursday, October 1st-Wednesday October 7th.  THERE WILL BE A DROP OFF OUTSIDE OUR OFFICE BETWEEN THE HOURS 8:00 and 4:00.  Please place your signed page in the slot on the box.   If you do not have access to a printer, there will be forms available to you. Keep the information pages and ONLY return the signature page.Your form MUST be returned prior to your student arriving on campus on October 12th. If you do not wish to sign this form, your child will be placed in a virtual class. Based on availability, this class may not be with a Los Peñasquitos teacher.
