2020-2021 School Year Enrollment
Enrollment for New Students
To enroll your child for school at Creekside, begin by completing the Enrollment Intake Form found HERE. This form will take approximately 15 minutes and you will be required to upload copies of your child's birth certificate, documents verifying your residency, and the parent's identification.
This is the first step of the enrollment process. Once your child's birthday and residency is verified by our school staff, you will be emailed a link to Step 2 of the enrollment process. Step 2 will require additional forms to be completed as well as uploading your child's immunization records.
PLEASE NOTE: Your student’s enrollment ensures a seat at a school in Poway Unified. We make every effort to enroll your child at your neighborhood school. In some cases, classes at your neighborhood school may reach capacity, in which case we will work with you to place your child at a nearby school.

For ESS Enrollment (Extended Student Services), contact ESS Supervisor, Gracie Degnan, at adegnan@powayusd.com to have your child added to the ESS Waitlist.
- Name of Child
- Date of Birth
- Name of immunization administered
- Month and year of EACH immunization. Include day if obtained within birth month.
- Name of health care provider or agency administering EACH vaccine.
- If you are taking your child to the doctor for their five year old physical and immunizations prior to enrollment, you may download the Report of Health Checkup for School Entrance MCH77 from the Poway Unified School District website (https://www.powayusd.com/enrollment/) under new student enrollment. The Oral Health Checkup Form is also available on the website.

Transitional Kindergarten (TK) Information
TK is the first year of a two-year kindergarten program that uses a modified kindergarten curriculum that is age and developmentally appropriate. The program allows students the time to mature socially, emotionally, and academically. Pursuant to law, EC 48000(c), a child is eligible for TK if a child will have his/her 5
th birthday between September 2 and December 2 (inclusive). TK is part of the public education system of Poway Unified School District and there is no cost to attend.
TK follows the same school year (start date, vacations, professional growth days, etc.) as the K-5 students except that TK is a half-day program and ends at 1:05 p.m. every school day at Creekside. [Fee-based Extended School Services (ESS) program is available for TK students who need before and/or after-school care. ESS hours are 6:30 a.m. to 6:00 p.m. on school days.]
TK Interest Wait List Information: Parents of children with a fifth birthday between June 1st and September 1st, 2020 who wish to enroll their child into a TK class:
- Enroll your child for Kindergarten at your school of residence
- Once enrolled, you will be given information regarding the TK Interest Waitlist
- Please note: TK space is very limited. Placement in TK is not guaranteed.

PUSD is aware of the amendment to California Education Code 48000(c) which permits districts the option to enroll students who turn 5 years of age after December 2 to be admitted to the Transitional Kindergarten program. After a review of this option, our District enrolls only students who meet the cutoff date of December 2 into the Transitional Kindergarten program.
