Kindergarten Enrollment
You
can download the 2008/2009 enrollment forms and complete them
prior to the registration date.
They are available on the
PUSD website now. Just go to the PUSD home page
and click "New Student registration" on the left of your
screen.
PLEASE NOTE:
Beginning Wed., June 25, 2008 and continuing through
Thurs., July 31, 2008 new student enrollment for
Kindergarten through 5th grade will be taking place at
the District Office.

Please
arrive with your enrollment packet, complete
immunization records, proof of student’s age and two
proofs of residency. With everything signed and
completed, you will be enrolled for the 2008-09 school
year.
Please use this form to tell us about your
Kindergarten student. Kindergarten Request form
and return it with your enrollment packet.
Incomplete packages will not be accepted! Kindergarten is All Day for all Kindergarteners. . . 8:00
am – 2:20 pm
Residency:
2 separate forms of residency, i.e. lease agreement,
bank statement,
utility bill.
Age
Requirement:
Must be five years of age on or before December 2,
2008.
Proof
of Age:
Birth Certificate, Baptismal Certificate or Passport.
Immunizations: See Poway website for list.
Physicals:
required for entrance into first grade and must be dated
within 18 months prior to 1st grade.
