TICKET TO PLAY

To get your “TICKET-TO-PLAY”, print and fill out all pages of Athletic Screening History & Physical Exam listed below. All Ticket-to-Play forms must be updated each school year. Physicals are good for ONE year from date of examination and cannot expire during the sports season. Only PUSD Athletic Screening History & Physical Exam forms are accepted. Bring ALL signed forms to the Athletic Office to receive your ticket:

• Athletic Screening History & Physical Exam

• Athletic Handbook

• Authorization for Medication form

• Physical Locations

• Transportation Policy

• Alternate Transportation Request Form
This form needs to be filled out BEFORE an athlete is allowed to take alternative transportation to or from a sporting event.
Directions for filling out this form -
1. Parent - fill out this form and sign it.
2. Student - take the form to your coach for signature.
3. Student - take the form to the Athletic Director for signature.
