Registration Dates
The following forms/documents were distributed during pre-registration at the end of the 2012-2013 school year for existing students: Student Information Form (personalized contact information), PUSD Signature Verification of Receipt of Documents and WHS Consolidated Signature Form. If these documents were already returned, they will not need to be submitted again at registration during August. Below are additional mandatory and optional forms, as well as documents to read only.

Student Information Form These pre-printed forms were distributed to incoming freshmen and returning students during the end of the 2012-2013 school year. If you are a new student, you will receive your form from the office (Wolverine Center).
