It is very important that the items listed above be completed, signed, and returned to the school.  The PUSD Student Registration Information Form should be completely updated prior to signing it.  Please be certain that all of the forms are signed by a parent or guardian, and the student has also signed in the appropriate places.  In addition, please be sure to include your e-mail address so that we can forward you our weekly updates.

Your student will receive our time management system, Oak Valley Student Planner, during the first week of school. Please review this tool carefully with your student and have your student bring it with them to school each day. Always contact the teacher if you have concerns or information that will support student success. 

 

 

If you decide to purchase Student Accident Insurance, fill in the application envelope, enclose check or money order, and promptly mail the self-addressed envelope.

 

We want your child’s experience at Oak Valley Middle School to be a positive team effort. Good lines of communication are an important part of this effort.  Parents and students are kept informed about current school events as well as changes or additions to the calendar via weekly family e-mail updates. Please be sure to update your e-mail address on the PUSD Student Registration Information Form. Our automated calling system enables us to call home with a recorded message for important events.  Please note that because of the budget crisis, we will be sharing most of our information with you through our web page and e-mails.

 

Please be sure you and your child sign all of the appropriate forms and return them at your appointed time on registration day.
