                               KINDERGARTEN ENROLLMENT UPDATE (5/1/13)

Monterey Ridge is currently at capacity. As was the case last year, we will not be accepting enrollment forms on a first come – first served basis. For Kindergarten and Transitional Kindergarten enrollment, we will be implementing the following process:

All Kindergarten registration will be as follows:

Monday, May 6th 8:00 am - 2:00 pm
Tuesday, May 7th 8:00 am - 2:00 pm

You may complete and submit the enrollment forms any time on these dates during the times specified above.

The following documents MUST accompany the Registration Form:
Original and one(1) copy of Birth Certificate
Original and one (1) copy of Immunization Records
2 Original Proofs of Residency plus one copy of each

The school will NOT be accepting forms on a first come –first served basis for TK and K.

During this registration period all students will be enrolled on a provisional basis. In the event we do not have enough space to enroll all Kindergarten students residing within the Monterey Ridge attendance area, the District may consider implementing a lottery or redirecting students to other schools in the area. Please continue to check the Monterey Ridge website as we will be posting updates about Kindergarten enrollment.

The Monterey Ridge office will be closed from Friday, June 21st thru Friday, August 9th.  The 2013-14 school year will begin August 21st, 2013.

During July, new student enrollment for Transitional Kindergarten through 5th grade will be taking place at the District Office. Check back for exact dates. Your child must be 5 years old on or before October 1, 2013.  Students with birthdays from October 2nd thru December 2nd must enroll in Transitional Kindergarten.

Please visit the Poway Unified School website at http://www.powayusd.com/enrollment/.  for the 2013-2014 enrollment forms.  Forms will be available on the District website May 1, 2013.

Required documents to bring from home at time of enrollment:

form Parent must bring their picture ID (i.e., Driver’s License)
formCompleted 2013-2014 enrollment packet, available online May 1st.
form Birth Certificate, Baptismal Certificate or Passport (Original and 1 copy)
form Social Security Number
formTwo (2) separate forms of residency verification (Original and 1 copy)
       (Example: deed to primary residence, lease agreement, utility bill.  PURCHASE
       AGREEMENTS WILL NOT BE ACCEPTED)
form Immunization Record (Original and 1 copy)

PLAN AHEAD
Immunizations Must Be Completed Prior to Enrollment

California immunization requirements are:

*Polio – 4 doses (3 doses if at least one was given after the fourth birthday)
*DPT –  5 doses (4 doses if last dose was given after the fourth birthday)
*MMR  Measles, Mumps and Rubella - 2 doses (both doses must have been given
  after first birthday)
*Hepatitis B – 3 doses
*Varicella (chicken pox) – 1 dose (OR health care provider documentation of
  Chickenpox disease)

Official immunization record must include:

  • Name of Child
  • Date of Birth
  • Name of immunization administered
  • Date of EACH immunization. Include day if obtained within birth month.
  • Name of health care provider or agency administering each vaccine.

If you are taking your child to the doctor for their five year old physical and immunizations prior to enrollment, you may download the Report of Health Checkup for School Entrance MCH77 from the Poway Unified School District website (http://www.powayusd.com/enrollment/) under new student enrollment. The Oral Health Checkup Form is also available on the website.

All children are required to have their vaccinations fully completed before enrolling in Kindergarten, unless exempted as provided by law.  Only two types of exemptions are allowed for immunization requirements:  (1) medical, or (2) personal beliefs.  Exemptions must be handled prior to enrollment.  Call the school office at 858-487-6887 regarding exemption forms.
