Forms are available in English and in Spanish.  Kindergarten enrollment will be May 2 thru May 11 from 9:00 a.m. to 2:00 p.m. Please download the forms, fill them out, and bring them to the office along with the following:

 

  • Original Birth Certificate or Baptism Certificate

  • Immunization Records

  • 2 Proofs of Residency

REQUIRED –
Documents must be original and immunizations must be up-to-date.
