2013-14 New Student Enrollment
Forms for New Student Enrollment for the 2013-2014 school year
will be available at the PUSD Enrollment link, on May 1, 2013.
Del Sur enrollment dates and times for the 2013 - 2014:
Enrollment for grades K - 5: May 10, 2013 (9:00 a.m. – 12:00 p.m.)
and continues through June 21.
Kindergarten /Transitional Kindergarten (TK)
Kindergarten orientation is tentatively scheduled for August (TBD) .
**NOTE: All children entering Kindergarten for the 2013/2014 school year MUST be five years of age on or before October 1, 2013. Students who enroll with birthdays from October 2nd thru December 2nd will be enrolled in TK. Click here for more information about Kindergarten and Transitional K.
Please visit the Poway Unified School website at http://www.powayusd.com/Enrollment/ after May 1st, 2013 for the 2013-2014 enrollment forms. Having the forms completed ahead of time and all documentation up to date is required to complete enrollment and will save time. TK/Kindergarten families will receive information in August which will indicate your child's teacher, room number and schedule.
REQUIRED DOCUMENTS TO BRING FROM HOME AT TIME OF ENROLLMENT
____ Birth certificate, Baptismal Certificate or Passport + 1 copy
____ Original Immunization Record + 1 copy
____ Two (2) separate forms of residency verification + copy of each form of residency
(Example: deed to primary residence, lease agreement, utility bill)

PLAN AHEAD…Have immunizations completed.
•4 Doses - Polio (3 doses if at least one dose was
given on or after fourth birthday)
•5 Doses - DPT (4 doses if at least one dose was given on or after fourth birthday)
•2 Doses - MMR (Both doses must be given on or after first birthday)
•3 Doses - Hepatitis B
•1 Dose - Varicella (Or health care provider documentation of chicken pox disease or immunity)
Official immunization record must include:
•Name of Child
•Date of Birth
•Name of immunization administered
•Month and year of EACH immunization. Include day if obtained within birth month.
•Name of health care provider or agency administering EACH vaccine.
If you are taking your child to the doctor for their five year old physical and immunizations prior to enrollment, you may download the Report of Health Checkup for School Entrance MCH77 from the Poway Unified School District website (http://www.powayusd.com/enrollment/) under new student enrollment. The Oral Health Checkup Form is also available on the website.
New Student Enrollment Grades K - 5
Enrollment for grades K - 5: May 10, 2013 (9:00 a.m. – 12:00 p.m.) and continues through June 21.
A birth certificate, current immunizations and 2 proofs of residency are required. Please go to the PUSD website to download the forms. Bring completed forms to your child's school to complete the enrollment process.
The Del Sur office will be closed from Wednesday, June 24th thru Friday, August 9th. School will begin the week of August 21st, 2013 - Please check the PUSD Kindergarten website for more details.
