Forms for New Student Enrollment for the
2013-2014 school year
will be available at the PUSD Enrollment link, on May 1, 2013.
Please note the immunization requirements below; students will not be permitted to enroll until these requirements are met. In addition, you must bring with you the child's birth certificate or passport, two separate forms of residency verification, and the completed immunization records.
Enrollment for students entering Kindergarten for the 2013-2014 school year will begin Tuesday, May 8th 8:00 am -11:30 am and continue weekdays through the end of school, June 21. Please review the immunization requirements below as these must be completed prior to your child's enrollment.
Enrollment for students entering 1st through 5th Grade for the 2013-2014 school year will begin May 15, 2013 (9:00 a.m. – 3:00 p.m.) and continue through June 21.
What to Bring to your appointment:
Grades K-5: Completed enrollment forms. You may download the forms on the PUSD link (above).
Kindergarten only: Your child's official birth certificate (we cannot accept the hospital certificate). Your child must 5 years old on or before December 2, 2009 (for 2009/10 enrollment).
Grades K-5: Your child's immunization record. Requirements are as follows:
- 5 doses DPT (4 doses if last dose was given after 4th birthday)
- 4 doses Polio (3 doses if last dose was given after 4th birthday)
- 3 doses Hepatitis B
- 2 doses MMR
- Varicella (chicken pox) immunization or physician's documentation that child has had the disease.
All children are required to have their vaccinations fully completed before enrolling in Kindergarten, unless exempted as provided by law. Only two types of exemptions are allowed for immunization requirements: (1) medical, or (2) personal beliefs. Exemptions must be handled prior to enrollment. Call the school office at 858-484-0981 regarding exemption forms.
Grades K-5: Two documents showing proof of residence. This can include utility bills, bank statement, lease or rental agreement, escrow papers. We cannot accept a driver's license or car registration as proof of residence.
We cannot enroll your child unless you have all of these documents with you at the time of registration. Please bring original documents with you, plus copies that we can keep.
