2009-2010 Enrollment for New Students

Creekside Elementary will begin kindergarten enrollment for the 2009 - 2010 school year on May 13, from 2:00-4:00 pm in the Multi-Purpose Room. 

All other grades (1st-5th) may enroll on May 14th or anytime thereafter in the school office between the hours of 9:30am – 11:30am until June 11. Kindergarteners must be five years old before December 2, 2009.

You must have the following information with you at enrollment (enrollment forms are to be accessed online at http://powayusd.com/enrollment/  and completed prior to the May 13th enrollment date): 

    • Certified birth certificate (not the hospital souvenir certificate or the request card to obtain the birth certificate) or passport or baptismal certificate.           
    • Social Security Number 
    • Two proofs of residency, ex: escrow papers, rental agreement, rent receipt, utility bills, bank statement, deed to home, property tax receipt.  (Check books and drivers licenses are not acceptable) 
    • Immunization records: Immunization record is a personal record with entries made by a physician or clinic, not dates from a baby book.  You must have the following to enroll: 
    • Polio – 4 doses at any age, but…3 doses meet the requirement if at last one was given after the 4th birthday.
    • DPT – At least 4 doses.  Last dose must have been given after the 4th birthday.
    • MMR (Measles, Mumps and Rubella) 2 doses, both doses must have been given after the first birthday.
    • Hepatitis B – 3 doses
    • Varicella (chicken pox) – 1 dose or health care provider-documented varicella disease or immunity
    • Completed 2009/2010 PUSD New Student Enrollment forms found on the http://powayusd.com/enrollment/ and completed Creekside enrollment forms found on http://powayusd.com/pusdcses/ .

 

 The State of California requires all 1st grade students to have a physical on file with the school (may be completed up to 18 months prior to or within 3 months of starting 1st grade) as well as an Oral Health Assessment (due on or before May 31st of their Kindergarten year). You can access both of these forms (CHDP & Oral Health Assessment) on the PUSD website. You may turn in both of these completed forms on May 13th (K enrollment) if you wish.
