NEW STUDENTS TO POWAY UNIFIED SCHOOL DISTRICT (PUSD)

(Continuing PUSD students, SKIP to Step 4.)

Please be sure to bring the following ORIGINAL documents with you upon enrollment:

1.    Original Birth Certificate or Current Passport or Baptismal Certificate.

2.    Immunization Records Click for current immunization information (K-12).  All children are required to have their vaccinations fully completed before enrolling in school, unless exempt, as provided by law.  PLEASE NOTE:  Students entering Grade 7 will need proof of a Tdap booster shot.

3.  Two documents as Proofs of Residency. (Click here for appropriate documents for address verification.)

Step 4

Ø  Return all necessary forms to New Directions at your enrollment appointment.

Ø  Please know that the enrollment process is not complete until you provide the school with all of the original documents (listed in Step 3) and all of the required forms listed below.

• Enrollment Form (required for new students to PUSD only)
• Courses Permission Slip
• Parent/Student Agreement
• Signature Verification Form
• Student Health Information (required for ALL grades)
