It is time to register your child for the 2008-2009 school year!  Once again, we will be offering early registration
during the month of June.  We’re asking our parents to download and complete the following forms from the
District/ABES website www.powayusd.com :

Click on Continuing Student à Download Signature Verification form

à Incoming 4^th grade students only, download Student Health Information form

 à Incoming 5^th grade students only, download California Healthy Kids Survey Elementary form

Please bring in the required forms (see above) to the office during the weeks of June 2^nd through June 13^th. 
Additionally, we will ask you to review and sign an updated Student Registration Information form.

**Each year, all PUSD families are required to complete the registration process**

If your child is attending Willow Grove, you will not be asked to complete the paperwork at our site.
