Kindergarten Enrollment for
Willow Grove Elementary School

WHEN:        February 28, 2008

TIME:          8:30 AM – 5:00 PM

WHERE:      Temporary Office for Willow Grove Elementary School located at Stone Ranch Elementary School in Room H6.

If you cannot enroll your child on February 28, enrollment will be ongoing through Friday, June 20, from 8:30 AM to 3:00 PM in Room H6 at Stone Ranch Elementary School.

Please note:  Your child must be 5 years old on/before December 2, 2008.

You must have the following information with you to register:

  • Original certified birth certificate or passport (Hospital souvenir certificate or the request card to obtain the certificate are not acceptable.)
  • Two proofs of residency such as escrow papers, rental agreement, rent receipt, utility bills, bank statement, deed to home, or property tax receipt (Check books and driver’s licenses are not acceptable.)
  • Social Security number
  • Immunization record:  This record should include the child’s name, date of birth, immunizations, and name of the health care provider.  To be current, the immunizations should include: 

ü      Polio – 4 doses (3 doses if at least one dose was given on/after the 4th birthday)

ü      DPT – 5 doses (4 doses if at least one dose was given on/after the 4th birthday)

ü      MMR (Measles, Mumps and Rubella) 2 doses, (must be given on/after the first birthday)

ü      Hepatitis B -  3 doses

ü      Varicella (Chicken pox) - 1 dose or health care provider-documented varicella disease or immunity.

All children must meet state-mandated immunization requirements prior to enrolling in kindergarten unless exempted as provided by law.
