Ticket-to-Play


Athletes need a “TICKET-TO-PLAY” to participate in sports. Print and fill out all pages of the Athletic Screening History & Physical Exam {link below}. All five pages of the physical form must be updated each school year. Physicals are good for ONE year from date of examination and cannot expire during the sports season {*}. Only PUSD Athletic Screening History & Physical Exam forms are accepted. Bring ALL signed forms to the Athletic Office. ID will be required to receive your ticket.

  • Complete the Athletic Screening History & Physical Exam
    • If you carry or use any medication while playing your sport, you must complete the Medical Authorization Form
    • {*} - if the student has been named to the team's roster and if the physical does expire during the season, it must be renewed as close to the expiration date as possible. Otherwise, the student will sit out until the new physical is received, reviewed, and approved.

Once you have been selected for a team the following forms are available {depending on need; not all students need to use these forms.}:
  • Athletic Exemption from PE Exemption is for 10th-12th grade students who are eligible to waive ENS {PE}.
  • Alternate Transportation Request Form - needs to be filled out at least two days BEFORE an athlete is allowed to take alternative transportation {separate from the team} to or from a sporting event.
  • Parent - Fill out this form and sign it.
  • Student - Sign form and obtain your coach's for signature.
  • Student - Have the Athletic Director sign.
  • Student - Give the form to your coach.
  • Private Vehicle Transportation for Students - Requires approval BEFORE a parent is allowed to transport athletes other than their own to or from a sporting event.
    • Parent - Fill out this form and sign it. Bring or email the form, insurance declarations page, and valid California drivers' license to the Athletic Assistant for approval; preferrably two days prior to the event.
