WHAT TO DO WHEN AN INJURY OCCURS?

Included in your choice to pre-designate a personal physician, you can now pre-designate a medical group.

You have the right to be treated immediately by your personal physician, (M.D., D.O.) if you notify your employer, in writing, prior to the injury. To qualify as your pre-designated personal physician, the physician must agree, in writing, to treat you for a work related injury, must have previously directed your medical care and must retain your medical history and records. Your pre-designated physician must be a family practitioner, general practitioner, board certified or board eligible internist, obstetrician-gynecologist, pediatrician or multi-specialty medical group whose practice is predominantly for non-occupational injuries or illnesses.

Employees can now pre-designate a medical group as long as the medical group serves as your primary “personal physician”. The medical group must be composed of MD’s and DO’s (no chiropractors) and be a multi-specialty medical group providing predominantly non-occupational medical treatment. (This would include Kaiser)

FREQUENTLY ASKED QUESTIONS:

  • What should I do if I am injured on the job?
  • Immediately notify your supervisor and report the injury. Report the injury even if you are not seeking treatment at this time.
  • Fill out the Employee Injury Notice your supervisor will provide.
  • If medical treatment is needed, you will be directed to an MPN provider, unless appropriate pre-designation is on file. You will be given a Treatment Referral Form giving authorization of treatment.
  • Employer/Supervisor will provide DWC-1 form:

  • Employer/Supervisor will go over the Supervisor’s Report with employee.


  • What should I do if I am injured on the job and my supervisor is not available? (This could be after work hours or week-ends)
  • Report injury immediately to your site Workers' Comp Representative or Annette Davies, Workers' Comp Tech at the District Office at (858) 748-0010 ext. 2452. (Monday – Friday 7:30 a.m. – 4:30 p.m.)
  • If it is not an emergency, report to your supervisor as soon as possible.
  • If you need medical treatment go to any Sharp Rees-Stealy between 8:00 a.m. and 8:00 p. m. daily.

    Directions to Rancho Bernardo Sharp:
    Off Highway 15 take the Rancho Bernardo Road exit.
    Go West on Rancho Bernardo Road.
    Left turn at Via Tazon.
  • If it is an EMERGENCY and you need medical treatment go to Pomerado Hospital.
    Once your condition is stable, contact your supervisor.
