Listed below are the necessary forms needed to complete your new student enrollment for the 2007-08 school year. 

SUMMER ENROLLMENT:

Elementary:  Beginning August 6, 2007, please contact your school of residence for enrollment dates and times.

Middle School:  Beginning August 6, 2006, please contact your school of residence to make an appointment to enroll your child.

High School:  Offices are open 8 am - noon throughout the summer.  Please call your school of residence to make an appointment  to enroll your child. 

Please pay close attention to the action required column as this will provide you the directions for completing this process successfully. 

STEP 1 ~ You have the option to download, read, and sign some District level forms OR fill-In, download, and sign District level forms.  It is very important that you read each of the listed forms.  When you sign the signature page, you are acknowledging that you have read all of the forms.

STEP 2 ~ Go to your student's school website(s) to obtain information about the 2007 - 08 school year and any additional site forms that may be required. Not all schools require additional forms.

STEP 3 ~ Return all necessary forms to your student's school(s) during assigned enrollment time.

STEP 4 ~ You will also need to bring the following ORIGINAL documents:  birth certificate, two (2) proofs of residency, and your student's immunization record.  (Grades 6-12 you will need to bring your child's transcript.  )
