To identify and locate the school
your child is to attend, please go to the Address
Lookup Tool.
This website provides all the necessary forms needed to complete your new student enrollment for the 2008-09
school year. Please follow the step-by-step instructions and proceed
as directed.
STEP 1
~ You will need the latest free version of Adobe Acrobat to
these forms.
Click
to download. Be sure to uncheck
Photoshop Album Starter Edition or any other add-in.
Once
download is completed, you may need to restart your computer.

It is important that you read each of the required
forms listed below. When you sign the Signature Verification
form you are acknowledging that you have read all
required forms.

~ Return all necessary forms to your student's school(s) during assigned enrollment time.

~ Please be sure to bring the following ORIGINAL documents with you
upon enrollment:
- Original Birth Certificate,
Current Passport
or Baptismal Certificate(K-12)

Immunization Records

vaccinations fully completed before enrolling in
school, unless exempted as provided by law.

Go to your student's
school website
to obtain information about the 2008 - 09 school year
and any additional site forms that may be required. Not
all schools require additional forms.
Please know the
enrollment process is not complete until you
provide the school with all required forms and the
original documents in Step
4.
If your child is in a Special Day
Class, please contact the Special Education Department
for placement at 858-668-4100.
