STUDENTS WHO WANT TO ATTEND WESTVIEW IN 2009-2010

(A)  New to PUSD and live within Westview boundaries:

New Student Enrollment - Please be sure to bring the following ORIGINAL documents with you upon enrollment: 

• Original Birth Certificate, Current Passport or Baptismal Certificate (K-12)

• Immunization Records
  Click for current immunization information (K-12). All children are required to have their vaccinations fully completed before enrolling in school, unless exempted as provided by law.

• Two Documents as Proofs of Residency  (K-12)
  Click for appropriate documents for address verification

• Transcript (6-12)

• Enrollment Form

• Home Language Survey

• Residency Verification

• Student Health Information

• Transfer of Student Records

(B)  New to PUSD and live outside of Westview boundaries:

Complete the Intradistrict Transfer Request, Form PP-126 and turn in at Westview;

or you may submit your application by visiting the Student Attendance Office
located at 13230 Evening Creek Rd., Ste. 207, San Diego, CA 92128.

After Approval … follow (A) NEW TO PUSD instructions above

(C)  Currently Attending a PUSD school and live outside of Westview
        boundaries:

Complete the Intradistrict Transfer Request, Form PP-126 and turn in at Westview; 

or you may submit your application by visiting the Student Attendance Office

If you are a “Transfer Student” and plan on participating on a Westview athletic team, complete the CIF Athletic Eligibility Verification form.
