WESTVIEW CHEER 2013-14

Application

Liability Release

Ticket to Play (proof of current physical on file with Westview HS)

Teacher Evaluations (Four total, all from current teachers. Evals should not be included with other paperwork in envelope, teachers will be mailing or routing to cheer box in WHS staff lounge.)

Copy of recent report card

All paperwork shall be placed in an envelope with student’s name on the outside and turned in to Mrs. Sharon Sharpe at the front office

The 2013-14 season options offered will be structured the same as last year:

**Tryouts for football season only

**Tryouts for football and basketball season

**Tryouts for basketball season only (Basketball tryouts will be held in November)

Important Dates:

Informational Meeting Agenda March 4 & 6, 2013

April 3, 2013 – Paperwork deadline

April 9, 10 & 11 6:00-8:00pm, Westview gym/dance room – Tryout workshops for Football Season or Full Season (Football and Basketball)

April 13, 2013 10:00 am, Westview gym – Cheer Tryouts for Football Season or Full Season (Football and Basketball)

April 15, 2013 – Mandatory parent/cheerleader meeting 6:00-8:00 pm, Westview Library
