Requirements
for Kindergarten Registration
The
child must be 5 years of age on/or before December 2, 2008.
Bring
the original Birth Certificate or passport and we will make a copy of it.
Bring
two Proofs of Residency which we will make a copy of (i.e.: SDG&E bill,
Cable bill, monthly bank statement).
Bring
your child's social security card.
Bring
your child's Immunization Card. All immunizations need to be up to date,
including their 5 year immunizations, before registration will be taken.
Immunization Requirements
for Incoming Kindergarteners

4 Doses, but... |
3 doses meet requirement if at least one dose was given on or
after the 4th birthday. |
| DTP |
5 Doses, but... |
4 doses meet requirement if at least one dose was on or after
the 4th birthday. |
Measles
Mumps
Rubella |
2 doses of measles and at least one dose of mumps and rubella |
Usually both given as MMR. Both must be on or after the
first birthday. |
| Hepatitis |
3 doses |
|
Varicella
(Chicken Pox) |
1 dose |
Or documentation from physician that the child has had disease. |
